Important Notice!

We need to have all the Permissions and Medical Releases submitted to the Stake as soon as possible.  These are important as we finalize our food menu and prepare for any special medical concerns to be aware of during the Trek.

fill one section out for each of your children that are attending the trek,

and return it to a member of your bishopric AS SOON AS POSSIBLE.
